All children enrolled at MMO must have the following forms – signed by parents – in their student files at MMO.

-Discipline and Permission forms must be updated each year.

-Medical forms must be updated every two years and include a vaccination record. 

-In addition, student files must contain a copy of the child’s passport or birth certificate,

and a copy of the DCFS Standards – signed by a parent.
